I hereby acknowledge and agree to the following terms and conditions as a participant in Sista Walk Association (SWA):
Waiver of Liability: I understand and acknowledge that participating in the fitness walking group involves certain risks, including but not limited to, risks of injury or harm from falls, slips, trips, collision with objects, and other accidents or incidents that may occur during the walking sessions. I voluntarily assume all such risks and agree to release, indemnify, and hold harmless Sista Walk Association, its organizers, and volunteers from all liability for any injuries, damages, or losses that may arise out of my participation in the walking group.
Medical Condition: I certify that I am in good physical health and have no medical conditions that would prevent me from safely participating in the fitness walking group. I understand that it is my responsibility to consult with a healthcare professional before participating in any physical activity if I have any concerns about my health or fitness level.
Photo Consent: I grant permission to Sista Walk Association to take and use photographs or videos of me during the fitness walking sessions for promotional purposes, including but not limited to, social media,
fliers, and other marketing materials. I understand that my image may be used without compensation and that SWA retains all rights to the photographs or videos.
Confidentiality: I understand and acknowledge that Sista Walk Association may share information about the fitness walking group, such as meeting locations and times, with other participants for the purpose of organizing and coordinating the group. However, I agree not to share or disclose any personal information about other participants without their explicit consent.
Agreement to Rules and Guidelines: I agree to abide by all rules, guidelines, and instructions provided by Sista Walk Association and its organizers during the fitness walking sessions. I understand that failure to comply with these rules and guidelines may result in my removal from the group.
